Physical Therapy Without a Referral in Indiana: 42 Days

The glass suite doors lining the corridor at FlexWerk in Carmel City Center

In Indiana you can see a physical therapist without a referral, and the therapist can evaluate and treat you for up to 42 calendar days from the start of care before a referral is needed to continue. That rule is why “do I need a doctor’s order first” has a different answer here than in some other states, and it matters to anyone in Carmel deciding whether the next appointment should be a clinician or a coach. FlexWerk’s suites are used by independent professionals who set their own services, and whether a licensed physical therapist currently practices from one of them is a question for coach matching and the host team; this page does not claim that one does.

What the rule says

Indiana Code 25-27-1-2.5 allows a physical therapist to evaluate and treat a patient without a referral for up to 42 calendar days from the date care begins. To continue treatment past that point, the therapist needs a referral from a physician or other authorized provider. Checked September 2026. This is general information, not legal or medical advice, and rules change; confirm current requirements with the therapist’s office or the state licensing board before relying on them.

Two things the rule does not decide. It does not decide what your insurance will pay for, since plan rules are separate from state law and some plans still ask for a referral for coverage; that is a question for the plan. And it does not decide whether physical therapy is the right first stop. It only removes the paperwork gate for the first six weeks.

Who the 42 days are built for

Direct access exists for the person with a nagging, non-emergency problem: a knee that complains on stairs, a shoulder that has stopped reaching the top shelf, a back that flares every time yard work starts. Rather than waiting weeks for a physician visit to obtain an order, that person can book a therapist directly, get evaluated, and begin treatment while the clock runs. If the problem resolves inside 42 days, the episode ends. If it needs longer, the therapist’s evaluation becomes the basis of a referral request, and the physician visit happens with the diagnosis already framed.

Anything that looks like an emergency, sudden severe pain, numbness spreading down a limb, a fall with a possible fracture, unexplained weakness on one side, is not a direct access question. That goes to a physician or urgent care first, every time.

Where a personal trainer fits, and where one does not

A trainer is not a substitute for any of the above. Indiana does not license personal trainers, and no coach, however experienced, diagnoses or treats pain. The clean division is laid out in the difference between a trainer and a physical therapist: the therapist restores function during an episode of care; the coach builds capacity after discharge. In the private-suite setting that handoff has a specific shape. Once a therapist clears you, a coach at FlexWerk works one on one in a reserved room, keeps the home exercise program as the warm-up, loads the uninjured patterns hard and the formerly injured one patiently, and sends you back to the clinician the moment something flags. The hiring checklist for that coach is in finding a personal trainer after physical therapy in Carmel.

Some licensed physical therapists run cash-based, appointment-only practices out of private training rooms rather than clinics, and a suite with a rack, cables and a closed door suits that model. Whether any such therapist currently works from a FlexWerk suite is exactly the kind of roster detail that changes, so treat it as an open question until the host team answers it.

The practical sequence for a Carmel reader

If something hurts now, use direct access: book a physical therapist, no order required, and let the 42 day window do its job. If you are cleared and want to get stronger, book the free consultation, bring the therapist’s written guidance, take the free InBody body composition scan and get matched with a coach who treats that guidance as the program’s boundaries. If you are unsure which you need, the physician is the right triage desk, and a good coach will tell you the same before you finish the sentence.

Six weeks without a referral is a useful rule. Knowing which professional to spend them with is the part that decides the outcome.

Related questions

What happens after 42 days of physical therapy without a referral?

The therapist needs a referral from a physician or other authorized provider to keep treating you. In practice the therapist's evaluation travels with the request, so the physician visit starts with the picture already drawn. Confirm the current rule with the clinic, since regulations change.

Does direct access mean my insurance will cover it?

No. The state rule removes the referral requirement for the first 42 days of care; it does not bind your insurer, and some plans still require a referral for coverage. Ask the plan before the first visit.

Can a FlexWerk coach treat an injury while I wait for physical therapy?

No. Coaches build strength in cleared clients and do not diagnose or treat pain, and Indiana does not license personal trainers. Use direct access to see a therapist first, then bring the discharge guidance to a coach.
