HSA Eligible Fitness Expenses: What Counts and What Does Not

Fitness spending is HSA eligible only when it treats a specific condition that a physician has diagnosed. The sorting test for any fitness receipt

Run the receipt through four questions, in order. A “no” at any step means treat it as not eligible until a professional says otherwise.

  1. Has a physician diagnosed a specific condition?
  2. Did the physician recommend this particular program or service to treat it, rather than exercise in general?
  3. Is the spending incurred for that treatment, not for general health, appearance or athletic goals?
  4. Do you hold the documentation, a letter and an itemized invoice, that ties the first three together?

What generally does not count

These land on the “no” side because they are general health or lifestyle spending:

  • Gym or studio membership dues for general fitness.
  • Personal training aimed at “getting in shape,” losing a few pounds or building muscle.
  • Group classes, race fees and sports league fees.
  • Wearables and apps sold as wellness products.
  • Supplements and protein products taken for general wellness.

If you have an HSA and have been using it at the pharmacy, it is tempting to assume anything health-adjacent is fair game. The IRS framing is narrower, which is why the question comes up so often.

What can qualify, and why

The thread running through eligible spending is that a clinician treats a diagnosed problem and the expense is part of that treatment.

  • Care from a licensed clinician for a diagnosed condition, because the expense is medical treatment itself.

The pattern is the same each time: look for the diagnosis, the physician’s recommendation, and spending directed at treating that diagnosis. When any of the three is missing, treat the expense as personal.

Mistakes people make with HSA fitness spending

  1. Assuming a doctor’s verbal suggestion is enough. Get it in writing, in terms of the diagnosis.
  2. Paying first, asking later. The order should be physician, administrator, then purchase.
  3. Mixing receipts. Keep medical and lifestyle spending in separate folders so a mistaken claim does not contaminate the file.
  4. Treating employer perks as HSA money. A wellness stipend follows different rules; the employer reimbursement route is a different conversation, and it is often taxable.
  5. Forgetting to re-check. Plan rules and administrator policies change; confirm each year.
